1609-PR Image

Per Surplus Lines Law Section 1609, a written declaration from the producer is required (1609-PR). The surplus lines licensee must upload a signed 1609-PR image when submitting a 1609-SLL filing. PSLA reviews the uploaded 1609-PR image to insure it is consistent with the 1609-SLL data entered into EFS. If discrepancies are found, the filing state is set to “Filing Returned” in EFS (Electronic Filing System) and an email notification is sent to the Filing Return contact listed in EFS. The agency must revise/submit a corrected 1609-SLL filing with associated 1609-PR image.

Below is a list of common discrepancies with submitted 1609-PR images.

  1. The License Number of the Individual Producer is not correct (use PID’s Search for Producer to correct).
  2. The Name of the Individual Producer is missing.
  3. The Signature of the Producer is missing.
  4. The image is not legible or PSLA cannot view it.
  5. The attached image is not a 1609-PR.
  6. The policy effective date on the PR image does not match the date entered in EFS.
  7. The policy expiration date on the PR image does not match the date entered in EFS.
  8. The Policy Number or Customer ID on the PR image does not match the data entered in EFS.
  9. The Type of Coverage (Kind) on the PR image is missing or does not match the one entered in EFS.
  10. The Description of Insured’s Operation (Class) on the PR image is missing or does not match the one entered in EFS.
  11. The Property or Casualty Limits on the PR image does not match the one entered in EFS.
  12. The Location of Risk cannot be a P.O. Box.
  13. The Location of Risk on the PR image is missing or does not match the one entered in EFS.
  14. The Type of Coverage (Kind) is not eligible for the limits that you have selected.
